Concurrent Documentation, sometimes referred to as Collaborative Documentation, is a process in which clinicians and clients collaborate in the documentation of Progress Notes, the Assessment, and Treatment Planning at the end of session/appointment. It offers clients the opportunity to provide their input and perspective on services and progress enhancing their sense of participation in treatment. Clients report increased engagement. CD is client/family centered and has the potential of increasing our cultural competence with clients/families by engaging in a constant feedback about what is working or not for them. CD allows the clinician to maintain the focus on what recovery and wellness topics are most important for the client. Benefits that clinicians and agencies have reported: CD eliminates the staff’s “treadmill” of always having to catch up on documentation of services, it improved the quality of life of staff, enhances compliance with documentation submission standards and increases capture of funding, increases capacity, and improves outcomes and consumer satisfaction with the services provided.
